After a certain amount of time has passed since the opening of our clinic, we have gradually become well known to the public.
Unfortunately, many of the patients who come to our clinic are patients who have been treated at other clinics but have either not been cured or have been refused treatment.

Our clinic specializes in oculoplastic surgery, which is performed on the eyelids and orbits around the eyes, and we have performed more than 10,000 surgeries per year (number of eyes, total of 4 clinics: from January to December, 2023).
I believe that this number is unparalleled in Japan and the world.

By taking advantage of this experience, we may be able to improve something at our clinic even for patients who were thought to be no more cured by surgeries performed at other hospitals.

Fat repositioning from the skin

This is a patient who had undergone a total of four cosmetic surgery procedures, including fat removal from the conjunctiva, fat removal through a skin incision, and fat removal from the conjunctiva again, at a well-known cosmetic surgery center.

The last surgery performed at Surgery B was able to remove only a small amount of fat (she was told it was the size of a small pea) due to scarring. She came to see us because she was worried about how to improve the situation.

There was clearly a large fat protrusion and both lower and upper eyelids were bulging.

In this case, it is not just the surface fat that is the problem, but also the fat behind it.

For this reason, I performed a deep orbital fat decompression surgery according to the decompression for patients with Graves' disease.

The entrance to the orbit was stiff with scarring from several previous surgeries, but as expected, the fat behind it (deep orbit) had not been manipulated, so it was relatively easy to remove.

Ptosis surgery (shortening of elevator muscle)

Since the shape of the eyelid became strange after the surgery, she underwent another revision surgery (skin excision) at the Plastic Surgery Department of Hospital C. However, the shape of the eyelid was still strange, so she came to our hospital hoping for a revision.

As one would guess, the patient had surgery performed by her previous surgeon.

I would guess that the heavy eyelids were created exactly as they were in the previous surgery, but the heavy eyelids were created too high. We often see patients in a similar condition after cosmetic or plastic surgery for droopy eyelids.

The position of the eyelid is not necessarily higher than the eyelid should be. If the eyelids are not in the proper position, the skin under the eyelids will sag, resulting in this condition.

The size of the black eye has increased, but the skin laxity has increased relative to the size of the eye, and the position of the eyelid is unnatural. Our surgery included additional skin excision, including the improperly positioned eyelid, and creation of a properly positioned eyelid.

The patient's eyes were open before surgery, but you can see the improvement after surgery.

Revision surgery

The patient came to our hospital for revision of an implantation surgery she had undergone a year ago at a major D cosmetic surgery center because the shape of her eyelids had become unnatural after the surgery. As in case 2, this patient also has sagging skin below the eyelid because the eyelid is positioned too high.

There is another problem. This is the protrusion of the skin at the inner corner of the eye, which is also known as the mongolian folds. This is also known as the Mongolian folds, and is more common among Asians, especially East Asians.

Because the Mongolian folds are stretched, no matter how beautifully the upper eyelid is made, it will be pulled downwards and will not result in a beautiful double eyelid line.

For this reason, this patient underwent an inner corner of the eye incision, upper eyelid skin excision, and heavy eyelid creation. After the surgery, a beautiful parallel heavy eyelid line was created and the patient's expression became natural.

Concerned about upper eyelid concavity

After undergoing surgery for a droopy eyelid at the Plastic Surgery Department of E University Hospital, he became concerned about the concavity of his upper eyelid.

After undergoing two surgeries at F Cosmetic Surgery in Tokyo to transplant fat from the groin into the upper eyelid, he came to our hospital for excision because his eyelid had bulged out this time. The previous transplanted fat protruded forward and gave her an unnatural look.

After excision of a small amount of skin and transplanted fat, a beautiful and natural looking eyelid was achieved.

Revision after excision of the skin above the left eyelash

She had left visual field disturbance due to left eyebrow ptosis as a result of left facial nerve palsy.

She had left eyebrow ptosis and a brow lift was desirable, but since the left upper eyelid skin had already been removed, a brow lift could not be performed alone, so a skin graft from the right upper eyelid was performed simultaneously.

Sequential surgery for the left lower eyelid ectropion and hyaluronic acid injections into the right laugh lines were also performed to improve her facial appearance.

Because the previous doctor removed the skin of the upper left eyelid, the amount of the upper eyelid differs greatly between the left and right eyelids.

After surgery, the appearance was considerably improved.

She underwent surgery to raise her left eyebrow, skin grafting from the right upper eyelid to the left upper eyelid, and an exenteration of the left lower eyelid.

She also had hyaluronic acid injections into her laugh lines.

Risks and incidence of treatment

• The surgery causes swelling of the wound and internal bleeding. The next day, the wound is very swollen. The swelling will improve about 80% in the first 2 weeks and will take about 6 months to completely disappear. Complete resolution of internal bleeding (bruising) takes about 4 weeks.
• As the eyelid muscles return to normal, tear absorption also returns to normal, so the eye often feels dry. It may also become impossible to close completely. In this case, eye drops or ointment treatment is necessary.
• Recurrence may occur.
• Surgery is performed to minimize the left-right difference as much as possible, but it cannot be completely symmetrical. If there is a large difference between the left and right sides after the surgery, sutures may be re-stitched or reoperation may be performed. The probability of re-suturing or re-surgery is 10%. In the case of insurance reimbursement, a fee will be charged each time the surgery is performed.
• The scars gradually become less noticeable after surgery, but scars may be visible and keloids may occur.
• After six months to a year, the muscles relax and the surgery may recur, in which case revision surgery may be necessary.
• All revision surgeries after cosmetic surgery are at your own expense.
• Surgery has its limitations and is never 100% ideal. (Just as you cannot keep getting 100 points on a test).
• If the wound is dissected after surgery, it will need to be sutured again.
• Orbital cellulitis may occur due to infection.
• Intraoperative sedation may cause hypotension, pulselessness, and respiratory depression.
